D형 간염의 원인

  • 급성 B형 간염이나 만성 B형 간염을 앓는 사람이나, 또는 B형 간염 바이러스 보균자가 D형 간염 바이러스에 감염될 때 생기는 간염을 D형 간염이라고 한다. 
  • B형 간염을 앓지도 않는 사람이나 B형 간염 바이러스를 보균하지도 않은 건강한 사람에게 D형 간염 바이러스(Hepatitis D virus)가 감염되어도 D형 간염이 생기지 않는다. 
  • 다시 말하면, D형 간염 바이러스 감염만으로 D형 간염이 생기지 않고 반드시 B형 간염 바이러스와 함께 감염되어야 D형 간염이 발병하게 된다. 

D형 간염의 감염경로 

  • B형 간염 바이러스가 있는 사람이 D형 바이러스를 보균하고 있는 사람의
  • 피나 점액에 접촉되거나, 
  • B형 간염 바이러스가 있는 사람이 D형 바이러스에 오염된 피나 피로 만든 약물 주사를 맞거나 
  • B형 간염 바이러스가 있는 사람이 D형 바이러스를 보균하고 있는 사람의 점막에 접촉하거나
  • B형 간염 바이러스가 있는 사람이 D형 바이러스를 보균하고 있는 사람이 썼던 불결한 바늘을 살균처리를 하지 않고 쓰거나 
  • B형 간염 바이러스가 있는 사람이 D형 바이러스를 보균하고 있는 사람과 성교를 할 때 감염될 수 있다. 

D형 간염의 잠복기

  • 잠복기는 2~8주 정도이다. 

D형 간염의 증상 징후

  • 아무 증상 징후가 없던 급성 B형 간염이나 만성 B형 간염을 앓는 사람이 D형 간염 바이러스에 감염되면 B형 간염이나 D형 간염이 추가로 발생할 수 있고 
  • 그의 증상 징후가 아주 심하게 급속도로 진행되어 짧은 시간 내에 사망할 수 있다. 
  • 즉 전격적 D형 간염이 생길 수 있다. 
  • 황달, 열, 간 비대, 복통, 구토, 기운 탈진, 입맛 상실 등의 증상 징후가 생길 수 있다. 
  • 때로는 사망할 수 한다. 

D형 간염의 진단

  • 증상 징후, 병력, 진찰 등을 종합해서 B형 간염과 D형 간염의 항원 항체 검사로 진단한다. 
  • D형 간염 바이러스 항체(Anti-HDV antibody) 검사, 방사선 면역 측정법 검사, 효소 면역 측정법 검사 등으로 진단할 수 있다. 
  • 또 그 외 다른 간염과 감별 진단한다.

D형 간염의 치료와 예방

  • 특효약이 없고 대증치료를 한다. 
  • B형 간염을 예방해서 간접적으로 D형 간염도 예방한다.
