페니실린 이외 약물에 의한 알레르기 Drug allergy other than penicillin

비 1형 IgE 중개 약물 알레르기가 생기면 구진 피부 발진, 다형 홍반 발진, 관절통, 구토 등의 증상이 생길 수 있고 1형 IgE 중개 약물 알레르기의 증상 징후보다 훨씬 덜 심한 증상 징후가 생길 수 있다.
이론상, 거의 모든 약물은 항원 역할을 할 수 있다. 약물 치료를 받을 때 약물 항원 항체 이상 반응이 생겨 약물 알레르기가 생길 수 있다.
때로는, 약물로 1형 IgE 중개 약물 알레르기가 생겼는지 비 1형 IgE 중개 약물 알레르기가 생겼는지, 약물 알레르기가 아닌 약물 부작용이 생겼는지(예, 아스피린 부작용으로 출혈이 생길 때처럼), 또는 약물 알레르기와 약물 부작용 두 가지가 동시 생겼는지 확실하게 감별할 수 없는 때도 있다.
이런 이유로 약물로 생긴 약물 알레르기와 약물로 생긴 약물 부작용, 또는 약물 유해 작용을 통틀어 그냥 “약물 부작용이 생겼다“고 말할 때도 많다.
앞서 이미 설명한 바와 같이, 페니실린이나 그 외 항생제, 신경 안정제, 마취제, 아스피린, 항히스타민제, 또는 요오드제 등 약물이 항원 역할을 할 수 있다.
약물 항원이 신체 내에 이미 있는 항체를 만나 항원 항체 이상 반응이 생기고 그로 인해 특이 면역 반응이 생길 수 있다. 이것이 약물 알레르기가 생기는 기전이다.
즉시 약물 알레르기의 예는 아나필락시스이고, 지연 약물 알레르기의 예는 혈청병이다.
때로는 약물로 약물 부작용이 생길 수 있고 항원 항체 이상 반응으로 약물 알레르기가 생길 수 있다.

• When a non-type 1 IgE-mediated drug allergy occurs, symptoms such as papular skin rash, polymorphic erythema rash, joint pain, and vomiting may occur, and symptoms may be much less severe than those of a type 1 IgE-mediated drug allergy.

• In theory, almost any drug can act as an antigen. Drug allergies can occur due to adverse reactions to drug antigens and antibodies when receiving medication.

• Sometimes, whether the drug has a type 1 IgE-mediated drug allergy, a non-type 1 IgE-mediated drug allergy, a non-drug allergy (such as when bleeding occurs due to an aspirin side effect), or a drug allergy and drug side effect. There are times when it is impossible to definitively discriminate whether the two occur at the same time.

• For this reason, it is often said that a drug-induced drug allergy, drug-induced drug side effects, or drug-related adverse effects are simply “there is a drug side effect.”

• As already explained earlier, drugs such as penicillin or other antibiotics, tranquilizers, anesthetics, aspirin, antihistamines, or iodine drugs can act as antigens.

• When a drug antigen meets an antibody already in the body, an antigenic antibody reaction can occur, resulting in a specific immune response. This is the mechanism by which drug allergies develop.

• An example of immediate drug allergy is anaphylaxis, and an example of delayed drug allergy is serum disease.

• Sometimes drugs can cause adverse drug reactions, and adverse reactions to antigenic antibodies can lead to drug allergies.
