천포창(펨피구스) Pemphigus

천포창(펨피구스) Pemphigus

  • 펨피구스는 항원 항체(IgG) 이상 반응으로 생기는 자가면역 대수포성 피부질환이다. 
  • 펨피구스를 천포창이라고도 한다.

천포창(펨피구스)의 종류

1. 심상성 천포창(심상성 펨피구스/ Pemphigus vulgaris)

2. 낙엽성 천포창 (Pemphigus foliaceous)

3. 양성 천포창 (Bullous Pemphigus) 등 여러 종류가 있다.

  • 여기서는 심상성 천포창(심상성 펨피구스/ Pemphigus vulgaris)에 관해서 주로 설명한다.
  • 처음에는 대수포가 구강 점막층에서 부터 시작할 수 있고 구강과 두피에서 시작할 수 있다.
  • 그 다음으로 몸통, 얼굴, 사지에도 생길 수 있다.
  • 이 병이 있으면 95% 이상, 구강 점막층에 물집이 생긴다.
  • 그 물집에 박테리아가 감염되지 않는 한 상흔이 생기지 않고 서서히 낫는 것이 보통이다.
  • 환부가 가렵고, 아프고 화끈 거린다.
  • 니콜스키 징후(Nikolsky’s sign), Asbaugh-Hansen 징후가 나타날 수 있다.
  • 임신부로 부터 받은 IgG로 이런 증상 징후가 신생아에게도 나타날 수 있다.

천포창(펨피구스)의 진단 치료

  • 병력, 검진 등으로 진단한다.
  • 화상을 집중적으로 치료하는 중환자 지중 치료실에서 특별 치료를 한다.
  • 수액과 전해질 공급 치료,
  • 프레드니손 치료, 면역글로불린 주사, Cyclophosphamide, Azapioprine, Mychophenolate 등 면역억제제로 치료한다.

Types of pemphigus 천포창(펨피구스)

  • Pemphigus is an autoimmune vesicular skin disease caused by an adverse reaction to antigen-antibody (IgG).

1. Pemphigus vulgaris (Pemphigus vulgaris)

2. Deciduous pemphigus (Pemphigus foliaceous)

3. There are several types, including Bullous Pemphigus.

• This article mainly discusses pemphigus vulgaris (Pemphigus vulgaris).

• Initially, vesicles may originate in the oral mucosa and may originate in the oral cavity and scalp.

• In turn, it can occur on the trunk, face, and extremities.

• In more than 95% of cases with this disease, blisters form in the oral mucosa.

• As long as the blister is not infected with bacteria, it usually heals slowly without scarring.

• The affected area is itchy, painful, and burning.

• Nikolsky’s sign and Asbaugh-Hansen’s sign may appear.

• With IgG from a pregnant woman, these symptoms may appear in newborns as well.

Diagnostic treatment of pemphigus 

• Diagnosis is based on medical history and examination.

• Special treatment in the intensive care unit, where burns are intensively treated.

• fluid and electrolyte supply therapy;

• Treatment with prednisone, immunoglobulin injections, cyclophosphamide, azathioprine, or mycophenolate, etc.
