척추 골절, Spine fractures (Spinal fractures/Fracture of spine/Vertebral fracture)

척주는 두개골 기저에서부터 미골의 맨 끝 척주까지 총 33개 척추뼈(Vertebra/척추골/척추)로 형성되어 있고
33개 척추골로 이루어진 척주는 위 아래 종으로 서로 단단히 인대로 결합되어 한 개의 척주(Spine)가 생긴다.
척주를 영어로 Vertebral column, 또는 Spinal column 이라고 한다.

해부학적 기능상 척주 분류

경부척추(경추/목뼈/Cervical vertebra),
흉부척추(가슴기둥/흉추/Thoraxic vertebra),
요부 척추(요추/Lumbar vertebra),
천골(엉치뼈/Sacrum),
미골(꼬리뼈/Coccyx)로 크게 5부분 척추로 나눠진다.

각 척추뼈의 뒤 부분에 한 개의 구멍이 있고 33개 척추뼈는 최하부에서부터 최상부까지 위아래로 차곡차곡 쌓여 한 개 척주가 형성된다.
그 척주의 중앙에는 열린 관이 있다. 그 관은 척주의 최상부에서 최하부의 끝까지 통해 연결되어 있다.
그 관속에 척수가 들어있다.
장난·운동·교통사고·낙상·안전사고 등으로 등뼈(척추뼈)나 팔뼈, 또는 다리뼈 등에 골절이 생길 수 있다(척주뼈, 등뼈 골절 참조).

척추 골절의 증상 징후

척주에 있는 33개의 척추(척추뻬/척추골) 들 중 어느 척추가 어느 정도로 골절되어 있는 지에 따라 척추 골절의 증상 징후가 많이 다르다.
척추가 심하게 골절되면 골절된 척추 부위에서 나온 척수 신경(Spinal nerve)도 손상될 수 있고,
골절된 척추의 바로 아래 부위 및, 또는 위 부위에 있는 척수 신경도 손상되고 마비될 수 있다.
척추 골절이 생겨 척수 신경 마비가 있으면 그 척수 신경 마비에 따른 증상 징후가 동시 생길 수 있다.

척추 골절의 치료

척추가 골절되었다고 의심되면 의료구급대나 병원 응급실, 또는 단골 의사에게 긴급전화진료 상담을 해서 그들의 지시에 따라 구급차나 다른 적절한 교통수단으로 종합 병원 응급 의료실로 속히 이송하여 응급 치료를 받아야 한다.
가능한 한 의사나 의료구급대원 이외 다른 사람이 척추 골절이 있는 환아를 골절된 현장에서 다른 곳으로 함부로 옮겨서는 절대로 안 된다.
특히 목에 있는 척추(경부 척추)가 골절되었다고 의심되면 의사의 지시 없이 목을 좌우로 또는 전후로 조금도 움직여서는 안 된다.
경추가 골절되었다고 의심되면 경부 척주의 양쪽을, 즉 목 양쪽을 무거운 모래주머니 등으로 목 부분을 고정해 목이 좌우상하로 움직이지 않게 목 부분을 고정시킨다.
그래서 목을 좌우로 또는 앞뒤로 조금도 움직이지 못하도록 응급 처치한다.
척추 골절이 있는 환아를 이송할 때는 환아의 머리끝에서 발끝까지 전신을 한 개의 나무토막을 움직이는 것같이 척주 전체가 조금도 꾸부러지지 않도록 해야 한다.
예를 들면, 의사의 지시가 없이는 큰 판자나 들것에 눕힌 채로 척주가 조금도 더 이상 꾸부러지지 않은 상태의 자세로 종합 병원 응급실로 데리고 간다.

상황에 따라 적절한 다른 응급처치를 할 수 있지만 가능한 한 척추 골절의 응급처치를 할 줄 아는 의사나 의료구급대원만 이런 응급처치를 해야 한다.
증상 징후, 병력, 진찰, 척추 X-선 사진, 척추 CT 스캔 검사 등으로 척추 골절을 진단하고 그 정도에 따라 수술치료 및, 또는 석고붕대 고정 등으로 치료한다.
소아청소년과 전문의, 소아 신경내과 전문의 및 소아 신경외과 전문의, 정형외과 전문의 등 여러 종류의 특수 의학과 전문의가 일개 치료 팀이 되어 서로 협력해서 치료하는 것이 이상적 치료이다.
교통사고, 추락사고, 또는 그 밖의 다른 종류의 사고로 신체 어디가 다쳤을 때는 경부 척추(목뼈)나 다른 척추에 골절이 생길 수 있고, 두개골 강 내나 복강 내 기관이 손상되어 그 부위에서 출혈이 생길 수 있고, 후 복강 공간 내 신장이나 복강 내 비장, 간장, 또는 위장관 등이 파열될 수도 있다. 그런 문제 함께 생겨 있나 알아보고, 있으면 적절히 응급 치료하는 것이 보통이다.

Spine fractures(Spinal fractures/Fracture of spine/Vertebral fracture) 척추 골절

The spinal column is made up of 33 vertebrae (Vertebra/vertebrae/vertebrae) from the base of the skull to the end of the coccyx.
The spinal column, which consists of 33 vertebrae, is the upper and lower spine and is firmly bonded to each other with ligaments to form one spine.
The spinal column is called the vertebral column or spinal column in English.

Classification of the spinal column by anatomical function

Cervical spine (Cervical vertebra),
Thoracic spine (chest column/thoracic vertebra),
Lumbar vertebrae (Lumbar vertebra),
Sacrum,
The coccyx (tail bone) is largely divided into 5 parts of the vertebrae.

There is one hole in the back of each vertebra, and 33 vertebrae are stacked up and down from the bottom to the top to form one spinal column.
At the center of the spinal column is an open tube.
The tube is connected from the top of the spinal column to the bottom of the spinal column.
The spinal cord is in the tube.
Fractures can occur in the spine (vertebrae), arm bones, or leg bones due to mischief, exercise, traffic accidents, falls, and safety accidents (see spine and spine fractures).

Symptoms, Signs of a vertebral fracture

Symptoms, signs of a vertebral fracture vary greatly depending on which of the 33 vertebrae (spinal vertebrae/vertebrae) in the spinal column is fractured and to what extent.
If the spine is severely fractured, the spinal nerve from the fractured spine can also be damaged.
Spinal nerves in the area just below and above the fractured spine can also be damaged and paralyzed.
If a spinal nerve palsy due to a vertebral fracture occurs, symptoms, signs of the spinal nerve palsy may occur simultaneously.

Treatment of vertebral fractures

If you suspect that your child’s spine has been fractured, you should consult a medical paramedic, hospital emergency room, or a regular doctor for emergency telephone consultation and, according to their instructions, be promptly transferred to the general hospital emergency room by ambulance or other appropriate means of transportation for emergency treatment.
As far as possible, no one other than a doctor or medical paramedic should move a child with a vertebral fracture from the fracture site to another location.
In particular, if you suspect that the vertebrae in the neck (cervical vertebrae) have been fractured, you should not move the neck from side to side or back and forth at all without a doctor’s instructions.
If it is suspected that the cervical spine is fractured, fix the neck part of the neck with heavy sandbags on both sides of the cervical spinal column, that is, both sides of the neck, so that the neck does not move left and right.
So, take first aid to prevent the neck from moving side to side or back and forth at all.
When transferring a child with a vertebral fracture, the entire spinal column should not be bent at all, such as moving a single piece of wood from the child’s head to toe.
For example, without a doctor’s instructions, your child is taken to the emergency room of a general hospital while lying on a large plank or stretcher and in a position where the spinal column is no longer bent.

Depending on the situation, other first-aid measures may be appropriate, but only doctors or paramedics who know how to provide first aid for spinal fractures should do such first aid as much as possible.
Symptoms, signs, medical history, medical examination, spine x-ray, spine CT scan, etc. to diagnose a vertebral fracture, depending on the degree of surgery, or fixation with a plaster bandage treatment.
It is an ideal treatment to treat pediatric and adolescent specialists, pediatric neurology specialists, pediatric neurosurgeons, orthopedic specialists, and so

on.

When the body is injured in a traffic accident, fall, or any other type of accident, a fracture may occur in the cervical vertebrae (neck bone) or other vertebrae, and bleeding may occur in the cranial or abdominal cavity due to injury.
The kidney in the abdominal cavity or the spleen, liver, or gastrointestinal tract in the abdominal cavity may be ruptured. It is common to find out if such a problem occurs together and provide appropriate emergency treatment if any.
