정신적 두통(정신성 두통/심인성 두통) Psychogenic headache

  • 두통의 원인들 중 가장 흔한 원인이 정신적 원인(심인성 원인)이다.
  • 요즘 같이 복잡한 세상에서 사는 성인들에게나 소아청소년들에게 정신적 두통은 아주 흔하게 생긴다.
  • 소아청소년 스스로 가정, 학교 또는 사회에서 일어나는 여러 가지 문제를 원만히 해결할 수 없을 때에 정신성(심인성) 두통이 생기기 쉽다.
  • 이런 복잡한 문제에 당면한 소아청소년들에게 정신적 문제가 생기기 쉽고 그로 인해 머리가 복잡할 수 있다.
  • 다른 어떤 문제로 걱정을 많이 하거나 정신적으로 불안할 때 정신적 두통이 생길 수 있고, 우울증이나 히스테리도 생길 수 있다.
  • 정신적 문제로 머리가 아픈 소아청소년들의 대부분은 학교, 가정, 사회생활을 원만하게 할 수 없는 때가 많다.

정신적 두통(정신성 두통/심인성 두통)의 진단

  • 병력, 증상 징후와 진찰소견 등을 종합해서 진단한다.
  • 심인성으로 머리가 아플 때는 진찰을 하고 다른 여러 가지 검사를 해 봐도 두통의 원인이 될 수 있는 기질적 병을 찾을 수 없을 때 정신성 두통이라고 추정 진단할 수 있다.
  • 따라서 두통을 일으킬 수 있는 기질적 병이 없다고 진단한 후 심인성 원인으로 머리가 아프다고 역으로 추정 진단한다.

정신적 두통(정신성 두통/심인성 두통) 치료

  • 정신성 두통을 일으킨 문제가 무엇인지 가능하면 우선 알아서 그 문제를 해결해 주는 방법으로 정신적 두통을 치료해야 한다.
  • 두통의 원인을 확실히 모르면 원인을 확실히 알 때까지 여러 번 반복진찰을 하고, 필요에 따라 임상검사를 반복해야 할 때도 있다.
  • 원인을 확실히 찾지 못할 때는 소아 신경내과 전문의, 소아정신과 전문의 또는 다른 여러 특수과 전문의의 의견을 들어가면서 진단 치료 할 수 있다.
  • 자녀가 정신적 문제로 머리가 아플 때, 때로는 부모들이 ‘꾀병을 부린다.’고 믿는 경우가 있다.
  • 그렇지만 그 자녀가 어떤 문제로 두통이 있는 것이지 꾀병을 하지 않는다.

 

Psychogenic headache 정신적 두통(정신성 두통/심인성 두통)

• The most common cause of headache is mental (psychological).

• Mental headaches are very common among adults and children and adolescents living in today’s complex world.

• Children and adolescents are prone to mental (psychiatric) headaches when they cannot solve various problems at home, school, or society.

• Children and adolescents faced with these complex problems are prone to mental problems and can lead to complex brain problems. • Anxiety or mental anxiety over any other problem can lead to mental headaches, depression or hysteria. • Most of the children and adolescents who have a headache due to mental problems often cannot make their school, home, or social life smooth.

Diagnosis of mental headaches (mental headache/psychiatric headache)

• Comprehensive diagnosis including medical history, symptoms, and examination findings.

• If your head hurts due to psychogenic pain, you can diagnose it as a mental headache when you cannot find an organic disease that may cause the headache even after having a medical examination and performing other tests.

• Therefore, after diagnosing that there is no organic disease that can cause headache, it is assumed that the head hurts due to psychogenic causes.

Mental Headache (Mental Headache/Psychogenic Headache) Treatment

• What is the problem that is causing the mental headache? If possible, you should first treat the mental headache in a way that will resolve the problem on your own.

• If you are not sure of the cause of the headache, you may need to repeat the examination several times until you know the cause, and then repeat the clinical test as necessary.

• When the cause cannot be determined, diagnosis and treatment can be performed with the opinion of a pediatric neurologist, a pediatric psychiatrist, or several other specialists.

• When a child has a head ache due to a mental problem, sometimes parents believe that they are’faking illness’.

• Nevertheless, the child is not faking sickness due to a problem with a headache.
