위 궤양 천공이나 십이지장 궤양 천공, Perforation of peptic ulcer of stomach or duodenum

대개의 경우, 위궤양이나 십이지장궤양을 적절한 약물로 적절히 치료하면 잘 나을 수 있다.
때로는 약물로 적절히 치료해도 궤양이 잘 낫지 않는다.
위궤양이나 십이지장궤양이 있으면 위나 십이장 관의 점막 층에 궤양이 생긴다.
그런 궤양이 제대로 치료되지 않고 점점 악화되면 위 점막 층이나 십이지장 점막 층이 점점 더 손상된다. 궤양이 점점 더 악화되면 점막 층 바로 아래에 있는 점막 층하 조직이 손상될 수 있다. 더 악화되면 점막 층하 조직 바로 아래에 있는 근육 층이 손상되고, 마지막으로는 위 벽이나 십이지장의 벽의 장막 층까지 궤양으로 손상될 수 있다. 이 지경에 이르면 위 벽이나 십이지장 벽의 일부가 완전 천공 될 수 있다.

일반적으로 소아청소년들의 위궤양이나 십이지장 궤양은 성인들의 위궤양이나 십이지장 궤양에 비해 잘 천공되지 않는다.
위궤양이나 십이지장궤양으로 위벽이나 십이지장의 벽이 천공될 때는 갑자기 배가 심하게 아프고 배 전체가 부으면서 복통이 계속 된다.
구토하고 금방 죽을 것과 같이 괴로워하는 것이 보통이다.
배를 살살 눌러 만지면 배 전체에 심한 압통이 생길 수 있다.
응급 수술 치료를 적절히 받지 않으면 사망하는 것이 보통이다.
병력 증상 징후 진찰 소견을 종합해 복부 X선 사진 검사, Ct 검사, 초음파 검사 등으로 진단할 수 있다.
수술치료 한다

In most cases, gastric ulcer or duodenal ulcer can be healed well if appropriately treated with appropriate drugs.
Sometimes the ulcer does not heal well even with proper treatment with medication.
If there is a gastric ulcer or duodenal ulcer, an ulcer develops in the mucous layer of the stomach or duodenum duct.
If such ulcers are not treated properly and get worse,
the gastric mucosa layer or duodenal mucosa layer becomes more and more damaged.
As the ulcer gets worse, it can damage the submucosa just below the mucous layer.
If it gets worse, the muscle layer just below the mucous layer tissue is damaged, and finally, the ulcer can damage the stomach wall or the serous layer of the duodenum wall.
At this point, the stomach wall or part of the duodenal wall may be completely perforated.

In general, in children and adolescents, gastric ulcers or duodenal ulcers are less perforated than gastric ulcers or duodenal ulcers in adults.
When the wall of the stomach or duodenum is perforated with a gastric ulcer or duodenal ulcer, the stomach suddenly aches severely and the entire stomach is swollen, and the abdominal pain continues.
It is common to suffer, such as vomiting and dying soon. Touching the stomach gently can cause severe tenderness throughout the stomach.
It is common to die without proper emergency surgical treatment.
