외이도 아토피성 피부염 Atopic dermatitis in the external ear canal

뾰루지, 피부 발진, 아토피성 피부염, 지루성 피부염, 또는 접촉성 피부염 등이 외이도 내 피부에 생길 수 있다.

아토피성 피부염이나 세균 감염으로 염증이 외이도 내 피부층에 생기면 외이도 속이 가렵다. 참고로 가렴증은 작은 아픔이다.
외이도 속이 가려울 때 면봉이나 성냥개비 등으로 외이도 속을 후비면 외이도 내 피부에 생긴 아토피성 피부염이 더 심해질 수 있고 또 거기에 세균이 감염될 수 있고 화농성 외이도염이 생길 수 있다. 그래서 가려움증이 더 심해 질 수 있다.
귓구멍을 후빌 때 외이도 내 외상을 입힐 수 있다.
외이도 내에 아토피성 피부염이 있는 아이들의 대부분은 신체의 다른 부위에 아토피성 피부염이 있는 것이 보통이다.
아토피성 피부염 이외 다른 여러 종류의 알레르기 질환이 있을 수 있다.

외이도 아토피성 피부염의 진단

병력, 증상 징후와 진찰소견 등을 종합해 이 병이 의심되면 이 직달경(귀 직달경)으로 외이도 속을 직접 들여다보고 진단한다.

외이도 아토피성 피부염의 치료

외이도 아토피성 피부염으로 귓구멍 속이 가려울 때 손가락 끝으로 귓구멍 입구와 이주 부분을 살살 문지르면 대부분의 경우 가려움증이 덜해진다.
뾰루지가 나있을 때 외이도를 살살 문지르면 몹시 아픈 것이 보통이다.
외이도 속에 상처를 낼 수 있고 외이도에 세균을 감염시킬 수 있기 때문에 성냥개비나 면봉 등으로 귓구멍 속을 후벼 주어서는 안 된다.
손가락 끝으로 귓구멍 입구를 살살 문질러 줄 때 귀가 아프거나 귓구멍 속이 계속 가려울 때는 의사의 진단 치료를 곧 받아야 한다.
외이도 속이 계속 가려우면 타이레놀이나 모트린 등 진통제를 1~2번 복용하면 가려운증이 임시로 덜할 수 있다.

그러나 이런 치료로 원인 치료가 되지 않으므로 결국 가려움증의 원인을 알아서 원인에 따라 치료 받아야 한다.
외이도 아토피성 피부염은 코르티코스테로이드 귀 점적제로 치료하면 잘 낫는다.
외이도 속 아토피성 피부염과 세균 감염성 외이도염이 동시에 생길 수 있다.
이때 외이도가 더 몹시 아프고 가려울 수 있다.
이때 세균 감염성 외이도염과 아토피성 피부염은 의사의 처방에 따라 DermOtic oil 귀 점적제, 코르티코스테로이드제와 항생제 성분이 든 종합 귀 점적제, 또는 Fluocinolone acetonide oil 0.0l% 귀약 등으로 치료 한다13.
외이도 아토피성 피부염은 자주 재발될 수 있다([부모도 반의사가 되어야 한다–소아가정간호백과]-제17권 소아청소년 피부질환–아토피성 피부염 참조).

Atopic dermatitis in the external ear canal 외이도 아토피성 피부염

Pimples, skin rashes, atopic dermatitis, seborrheic dermatitis, or contact dermatitis can occur on the skin in the ear canal.

• Atopic dermatitis or bacterial infection causes the external auditory meatus to itch when inflammation occurs in the skin layer of the external auditory meatus. By the way, itching is a small pain.

• When the inside of the ear canal is itchy, if you pick the inside of the ear canal with a cotton swab or matchstick, atopic dermatitis on the skin inside the ear canal can get worse, and bacteria can become infected there, and purulent otitis externa can occur. So, the itching can get worse.

• When the ear canal is pulled, it can cause trauma to the external auditory meatus.

• Most children with atopic dermatitis in the ear canal usually have atopic dermatitis in other parts of the body.

• You may have several types of allergic diseases other than atopic dermatitis.

Diagnosis of atopic dermatitis of the external auditory meatus

• If this disease is suspected based on the medical history, symptom signs, and examination findings, look directly into the external auditory canal with the autoscope

Treatment of atopic dermatitis of the external auditory meatus

• When itching inside the ear canal due to atopic dermatitis of the external auditory canal, gently rub the mouth and trabeculae with your fingertips to relieve itching in most cases.

• Gently rubbing the ear canal during pimples is usually very painful.

• Do not poke into the ear canal with a matchstick or cotton swab, as it can injure the ear canal and infect bacteria in the ear canal.

• If your ear hurts when you gently rub the opening of the ear canal with your fingertips or if the inside of the ear cavity continues to itch, you should seek medical attention immediately.

• If the inside of the ear canal continues to itch, taking an analgesic such as Tylenol or Motrin 1 or 2 times may temporarily relieve the itching.

• However, since these treatments do not cure the cause, eventually, the cause of the itch must be identified and treated according to the cause.

• Atopic dermatitis of the external auditory meatus can be cured by treatment with corticosteroid ear drops.

• Atopic dermatitis and bacterial-infectious otitis externa can occur at the same time. • At this point, the ear canal may be more painful and itchy.

• In this case, bacterial infectious otitis externa and atopic dermatitis are treated with DermOtic oil ear drops, comprehensive ear drops containing corticosteroids and antibiotics, or Fluocinolone acetonide oil 0.01% ear drops according to the doctor’s prescription13.
