어루러기(전풍)의 원인

  • 어루러기를 전풍이라고도 하고 Tinea versicolor, 또는 Pityriasis versicolor라고도 한다. 
  • 어루러기는 Pityrosporium orbiculare(Malassezia furfur)란 진균 감염으로 생기는 진균 피부염의 일종이다. 
  • 두부 백선, 체부 백선, 족부 백선, 손톱 백선, 결절 백선, 흑 백선, 수 백선, 안면 백선, 완 백선 등 여러 종류의 진균 피부 감염 중에서 가장 흔하다. 
  • 인설이 많이 생기고 1~3cm의 크기의 타원형의 반점이 상 체부, 등, 팔의 상부, 얼굴, 또는 다른 부위에 있는 피부에 생길 수 있다. 
  • 진한 갈색이나 엷은 황갈색, 연한 적색, 또는 백색으로 나타날 수 있고 원래의 피부색보다 더 진하게 또는 좀 더 엷게 나타날 수 있다. 
  • 15~30세 연령층에 더 잘 나타나나 노인들이나 15세 이전 사춘기 이전 소아청소년들에게도 나타날 수 있고 영아들에게도 나타날 수 있다. 
  • 만성으로 나타날 수 있고 여름철 햇볕에 쪼이면 더 밝게 보이고 겨울철 햇볕으로 더 검게 보일 수 있다. 
  • 대개 아무 증상 징후가 없으나 경미하게 가려울 수 있다. 

어루러기(전풍)의 진단

  • 병력, 증상 징후, 진찰소견 등을 종합해서 진단할 수 있다. 
  • 환부에서 인설을 채취해 현미경 검사로 진단하든지 우드 등빛으로 진단하든지 진균 배양검사로 확진할 수 있다. 
  • 백색 비강진이나 염증성 피부질환이 나은 후 생기는 색소 침착 감소증, 백반증, 지루성 피부염, 2차 매독, 장미색 비강진(Pityriasis rosea) 등의 피부병과 감별 진단해야 한다. 

어루러기(전풍)의 치료

  • 셀레니움(Selenium sulfate and propylene glycol) 샴푸로 잘 치료될 수 있다. 
  • 때로는 Clotrimazole, Haloprogin, Miconazole, 또는 Tolnaftate 등의 항진균제 연고 중 하나를 선택해 치료하면 잘 치료된다.

The cause

• It is also known as  Tinea versicolor, or Pityriasis versicolor.  Pityrosporium orbiculare (Malassezia furfur) is a type of fungal dermatitis caused by a fungal infection.

The most common of many types of fungal skin infections such as tinea head, tinea body, tinea foot.

• A lot of scales are formed, and oval spots of 1 to 3 cm in size may appear on the skin on the upper body, back, upper arms, face, or other areas.

• May appear dark brown, pale tan, pale red, or white, and may appear darker or lighter than the original skin color.

• It is more pronounced in the 15-30-year-olds, but it can be seen in the elderly, pre-adolescent children before 15 years of age, and it can also be seen in infants.

• It may appear chronic and may appear brighter in the summer sun and darker in the winter sun.

• Usually there are no signs of symptoms, but it may be mildly itchy. Diagnosis 

• The medical history, symptoms, symptoms, and examination findings can be combined and diagnosed.

• It can be confirmed by a fungal culture test, whether it is diagnosed by microscopic examination by collecting scale from the affected area or by using a wood lamp.

• Differential diagnosis should be made from skin diseases such as hypopigmentation, vitiligo, seborrheic dermatitis, secondary syphilis, and Pityriasis rosea, which occur after healing of white nasal inflammation or inflammatory skin disease.

Treatment 

• It can be treated well with Selenium sulfate and propylene glycol shampoo.

• Sometimes treatment with an antifungal ointment such as Clotrimazole, Haloprogin, Miconazole, or Tolnaftate works well.
