안와 봉소염Orbital cellulitis – 제1권. 소아청소년 응급의료

안구와 안구에 관련된 혈관, 신경, 근 등을 둘러싼 골성 강을 안와라고 한다. 그 안와 내 염증을 안와봉소염이라 한다.
임상적으로 안와봉소염과 안와 주위염을 감별 진단하기가 때로는 어렵다.
안와봉소염이 생기면 안구가 밖으로 밀려 나올 수 있고 결막이 붓고, 안구를 좌우 상하 방향으로 자유자제로 움직이기 어렵고, 안검도 붓고 발적될 수 있다.
열이 나고 독성상태에 빠지기도 한다.
원인으로서는 부비동염을 일으켰던 연구균, 폐렴연쇄상 구균, 포도상 구균, 히브균(HIV)균 등이 안와에 퍼져서 생길 수도 있고,
안검, 안구, 결막, 누낭, 또는 누선에 염증을 일으켰던 박테리아가 안와로 퍼져 안구염을 일으킬 수도 있다.
패혈증을 일으켰던 박테리아가 안와에 퍼져 안와 봉소염이 생길 수 있다.
안와 외상 등으로 안와 봉소염이 생길 수 있다.
안와 봉소염이 점점 더 진행되면 안와 골막염, 시신경염, 뇌막염 등이 생길 수 있고 안와내 농양도 생길 수 있다.
병원 입원 광범위 항생제 혈관주사로 치료하고 필요에 따라 수술로 배농 치료한다.
