신생아 테타니, Neonatal tetany

저칼슘혈증이란 제목 하에서 이미 설명했지만 여기서 테타니란 제목으로 저칼슘혈증에 대해서 설명하기로 한다.
신생아의 혈중 칼슘의 농도가 정상치보다 현저히 낮을 때는 경련 등 여러 가지 증상 징후가 생길 수 있다.
이렇게 신생아의 혈중 칼슘 농도가 정상 이하로 낮을 때 생기는 경련을 신생아 테타니라고 한다.

신생아 테타니의 원인

체중이 아주 적은 미숙 신생아
만삭에 태어났지만 체중이 비정상적으로 아주 가벼운 신생아(저체중 신생아)
1형 당뇨병이 있는 임신부로부터 태어난 신생아
어떤 병을 앓고 있는 신생아
칼슘이 충분히 들어 있지 않은 인공영양물을 먹는 신생아
신장 기능이 저하되어 있을 때
부갑상선 기능 저하증 등이 있는 신생아에게 신생아 테타니가 생길 수 있다(p.000 신생아 저칼슘혈증 참조).

신생아 테타니의 증상 징후

혈중 칼슘 농도, 테타니의 정도, 원인 등에 따라 증상 징후가 다르다.
신생아 테타니의 전형적인 증상 징후는 다음과 같다.
보채고 조그마한 자극에 깜짝깜짝 잘 놀란다.
몸과 팔다리를 부들부들 떨 수 있고,
더 심할 때는 전신경련을 한다.
테타니의 증상 징후는 심한 저혈당증의 증상 징후와 거의 비슷할 때가 많다.

신생아 테타니의 진단

병력·증상 징후·진찰소견 등을 종합하여 이 병이 의심되면 칼슘·인·혈당 등의 혈중 농도를 측정하여 진단한다.
근본적인 원인이 무엇인지를 알아야 한다.

신생아 테타니의 치료

테타니의 정도·증상 징후·원인에 따라서 치료한다.
증상 징후가 경미할 때는 의사의 처방에 따라 경구용 칼슘 글루코네이트로 치료한다.
전신경련을 동반한 테타니는 중환자 집중 치료실에서 칼슘 정맥주사로 치료한다.
칼슘 정맥주사로 치료하는 동시에 경구용 칼슘으로 치료한다.
칼슘 정맥주사로 치료할 때 심장마비를 일으킬 수도 있다. 병원 중환자 집중 치료실에서 심전도 검사로 심장을 모니터하면서 치료하는 것이 원칙이다.
또 칼슘을 정맥주사로 주는 도중 칼슘이 정맥 주의 조직으로 새어나올 수 있다. 이 때 칼슘이 정맥 주사 주는 살 속에 침전될 수 있다.
다시 설명하면 칼슘 정맥주사 치료를 할 때는 병원에서 심전도 검사를 하면서 심장 기능 상태를 관찰하면서 의사가 칼슘 정맥주사를 주어야 한다.
