신생아 출혈이 생길 때, Neonatal bleeding

갓 태어나서 생후 1주경까지 거의 모든 신생아들의 체내 비타민 K 농도가 생리적으로 낮아 비타민 K 결핍증이 있을 수 있다.
비타민 K는 피를 응고시키는 데 꼭 필요한 혈액응고 요소 중 하나이다. 비타민 K가 많이 결핍되면 신체 어디에서든지 자연 출혈이 생길 수 있다. 그런 자연 출혈을 예방하기 위해 갓 태어난 모든 신생아들에게 비타민 K 주사를 통상적으로 놓아 준다. 특히 갓 태어난 미숙 신생아들이나 어떤 병을 앓는 신생아들에게 비타민 K를 주사를 좋아 출혈을 예방하는 것이 보통이다.
분만 중 질 산도 속에서 모체의 피를 삼킨 후 태어난 갓난아기가 피를 토하는 것이 보통이다. 모체가 흘린 피를 많이 삼키면 그 피의 일부를 토할 수 있고 그 피의 일부나 전부가 위 장관을 거쳐 대변과 같이 항문으로 배설될 수 있다.
이 때 신생아의 대변의 색이 빨갛거나 커피색 같이 적갈색일 수 있다([부모도 반의사가 되어야 한다– 소아가정간호백과]-제5권 인공 영양, 비티민, 민네랄.-p. 비타민 K 참조).
모유수유 중 엄마의 젖꼭지에서 피가 날 수 있다. 그런 젖꼭지를 빨아 젖을 먹을 때 젖 먹는 아기가 엄마의 피를 삼키고 그 피를 토할 수 있다.
갓 태어난 신생아의 구토물이나 대변에 섞여 나온 피가 신생아 자신이 흘린 피인지 분만 중 또는 수유 중 엄마의 젖꼭지에서 나온 피인지 금방 쉽게 확실히 알 수 없는 때도 있다.
신생아의 구토물이나 대변으로 나온 피가, 모체 혈액 감별 임상검사를 하면, 누구에게서 나온 피인지 비교적 쉽게 알 수 있다( 신생아가 피를 토할 때, 피를 토할 때 참조).
혈액응고 이상, 비타민 K 결핍증, 백혈병, 외상, 감염병, 우유 알레르기 등으로 신생아에게 출혈이 생길 수 있다.
신생아에게 출혈이 생길 때는 의사에게 곧 문의해서 진단 치료를 받아야 한다.
