분만이 시작되면서 아기가 완전히 배출 될 때까지 자궁이 수축되었다 이완되고 이완되었다 수축된다. 이런 자궁 이완 수축이 반복되면서 자궁 속에 있던 아기가 산도 속을 통해 산도 밖을 향해 점점 내려간다. 이 때 머리, 얼굴, 또는 엉덩이 중 한 부위가 산도 속으로 가장 먼저 나오는 것이 보통이다.
산도 속에 가장 먼저 나온 얼굴이나 머리 등의 부위가 산도에 눌릴 때 그 부위에 점상 출혈 반점이 생길 수 있다.
자궁이 수축 될 때 산도 속에 있는 아기의 머리나 얼굴 등에 상당한 압력이 가해지게 된다. 그래서 아기의 머리가 몰딩(주형)될 수 있고, 산류도 생길 수 있고, 또 산도 속에 있는 아기의 두피나 얼굴의 피부 등 선진부에 있는 모세혈관이 터질 수 있다.
갓 태어난 아기의 머리, 이마, 얼굴 등 부위에 있는 모세혈관이 터지면 그 부위에 점상출혈이 생길 수 있다. 이렇게 생긴 점상출혈은 마치 바늘로 피부를 꼭꼭 찔렸을 때 생긴 출혈 반점과 거의 비슷하다. 점상출혈로 생긴 반점들은 생후 2주 이내 자연히 없어진다.
어떤 감염병, 혈소판 저하증, 또는 전신 혈액응고 이상 등으로 점상출혈이 생길 수 있다. 영유아나 학령기 아이들이 심하게 토할 때, 또는 기침할 때도 주로 얼굴에 점상출혈이 생길 수 있다.
