시신경염(시각신경염) Optic neuritis

염증, 탈수질(수초 탈락), 퇴행변성 등이 시신경에 생길 때 시신경염이 생길 수 있다.
안구 내 유두염증이 있고 그 유두가 부을 수 있는 안구 내에 시신경염이 생길 수 있다.
또 시신경 유두는 정상적이고 안구 후부에 있는 시신경에 염증이 생길 수 있는 안구 후부 유두염도 있다.
시신경 유두에만 염증이 있는 시신경염을 유두염(Papillitis)이라고 한다.
때로는 유두염이 있고 망막에도 염증이 있을 수 있다.
이런 시신경염을 시신경 망막염증이라고 한다.

시신경염의 원인

유행성 이하선염,
홍역,
수두 등 바이러스 감염,
뇌수막구균 뇌막염 등 박테리아 감염
바이러스성 뇌막염
바이러스성 뇌염,
전신적 루프스,
파종상 경화증,
외부 독물,
약물중독,
납중독 등으로 시신경염이 생길 수 있다.

시신경염의 증상 징후

원인에 따라 다르다.
시력기능이 약해지거나 소실될 수 있다.
급성으로 시력상실이 점진적으로 생길 수 있다.
일측성이거나 양측성이다.
안구를 움직일 때 안구에 통증이 생길 수도 있고 안구를 살짝 누르면 아플 수 있다.
경미한 시신경염은 증상 징후가 시작한 후 1~4주 내에 점진적으로 회복되는 경우가 많다.
그러나 완전히 시력을 상실할 수 있다.

시신경염의 진단 치료

증상 징후, 병력, 검진 등으로 진단할 수 있다.
검안경으로 눈 안저를 들여다보면 시신경 유두(Optic disc)에 염증이 있는 것을 볼 수 있다.
유두가 부어있고 출혈반점도 발견될 수 있다.
치료는 원인에 따라 한다.

Optic neuritis 시신경염

• Optic neuritis can occur when inflammation, demyelination (myelination), and degeneration occur in the optic nerve.

• There is inflammation of the papillae in the eyeball and optic neuritis can develop in the eyeball where the nipples can swell.

• There is also posterior papillitis, where the optic disc is normal and the optic nerve at the back of the eye can become inflamed.

• Optic neuritis in which only the optic nerve papilla is inflamed is called papillitis.

• Sometimes there is papillitis and the retina may also be inflamed.

• This type of optic neuritis is called optic nerve retinitis.

Causes of optic neuritis

• Mumps; • measles;

• Viral infections, such as chickenpox;

• Bacterial infections, such as meningococcal meningitis • Viral meningitis

• viral encephalitis;

• systemic lupus,

• disseminated sclerosis;

• External poisons;

• drug addiction;

• Optic neuritis may occur due to lead poisoning.

• Depends on the cause.

• Vision may be weakened or lost. • Acute blindness may occur gradually. • It is unilateral or bilateral.

• When you move your eyeball, you may feel pain in your eyeball, or it may hurt when you lightly press on your eyeball.

• Mild optic neuritis often resolves gradually within 1 to 4 weeks after symptom onset.

• However, complete blindness can occur.

Diagnosis, treatment of optic neuritis

• It can be diagnosed by symptoms, medical history, and examination.

• If you look at the fundus of the eye with an ophthalmoscope, you can see that the optic disc is inflamed.

• Swollen nipples and bleeding spots may be found.

• Treatment depends on the cause.
