미숙 신생아 흡인성 폐렴, Aspiration pneumonia in premature infants

  • 대부분의 큰 미숙 신생아들은 모유나 인공영양을 만삭으로 태어난 신생아들과 거의 마찬가지로 잘 먹을 수 있으나 특히 아주 작은 미숙 신생아들의 일부는 모유나 인공영양을 먹을 때 모유나 인공영양을 입안에서 식도 속으로 정상적으로 삼킬 수 있는 힘이 약하기도 하고 때로는 삼킬 수 없다.
  • 또 태어날 때 침, 양수, 구토 물, 피, 태변 등을 입안에서 식도관 속으로 잘 삼킬 수 없어 그런 것이 기도 속으로 흡인되기도 한다.
  • 또 신생아들이나 특히 미숙 신생아들은 태어날 때 입안에 있는 피나 태변 등을 토해서 입 밖으로 내보내는 구토 반사가 미약하다.
  • 그래서 모체의 피나 자신이 싼 태변 등이 기도 속으로 흡인될 때도 있다.
  • 모유나 인공영양을 먹을 때 인공영양이나 모유가 식도관 속으로 정상적으로 넘겨 들어가는 대신 기도 속으로 잘못 흡인될 수 있다.
  • 이런저런 이유로 기도 내로 흡인된 인공영양이나 구토 물, 또는 태변 등으로 폐렴이 생길 수 있다.
  • 이렇게 해서 생긴 폐렴을 흡인성 폐렴이라 한다.
