달리기 스포츠로 인한 골단염 Apophysitis in running sports

근육과 건이 붙어 있는 뼈 부분을 골단(Apophyses)이라 한다(p.00 그림 참조).
그 골단에 생긴 염증을 골단염이라 한다.
골단은 근육이 수축되고 이완될 때 골단 손상이 생길 수 있다.
골단염이 생긴 뼈 부분이 은근하게 아프고 달리기를 하면 좀 더 아프다.
건이 부착된 골단 부분에 국소적 압통이 생긴다.
그 부분에 반상 출혈도 생기기도 한다.
골단염이 있는 신체 부분의 근력 제한이 생긴다.
골단염은 특히 전 상부 좌골 극에 더 잘 생길 수 있고, 슬개골 하극에도 잘 생길 수 있다.
슬개골 하극에 생긴 골단염을 신딩 랄센 조한스센 증후군(Sinding-Larsen-Johanssen syndrome)이라고 한다.
경골 조면에도 골단염이 생길 수 있다.
이 병을 오스굿-슐래터 병(Osgood-Schlatter disease, p.00)이라고 한다.
또 발의 종골 후면에도 생길 수 있다.
이 병을 종골 골단염(Calcaneal apophsitis) 또는 시버병(Sever’s disease)라고 한다.
중족골에도 골단염이 생길 수 있다.
제 5 중족골의 베이스에 생긴 골단염을 이셀린(Iselin) 병이라 한다.
장골능선(엉덩이뼈 능선)에도 골단염이 생길 수 있다. 장골릉 골단염(Iliac crest apophsitis)이 있으면 사두근이나 엉덩이 근육에 압통이 생기고 통증이 생길 수 있다.
장골능선에 생긴 골단염은 골단염이 생긴 부위에 따라 레스트, 얼음찜질, 신장 운동, 물리치료, 운동치료 등으로 치료한다. 때로는 스트랩 등으로 치료한다.

The part of the bone where muscles and tendons are attached is called apophyses .

Inflammation of the epiphysis is called osteoarthritis.

Epiphysis can be damaged when muscles contract and relax.

The part of the bone with osteomyelitis is subtly painful, and it hurts more when running. Local tenderness occurs in the epiphysis where the tendon is attached. An ecchymosis may also occur in that area. Restriction of strength in the part of the body with osteomyelitis.

Osteoarthritis is more likely to develop in the anterior superior ischial pole, and may also occur in the inferior patella pole. Osteoarthritis of the subpatella is called Sinding-Larsen-Johanssen syndrome. Osteoarthritis may also develop in the rough tibia. This disease is called Osgood-Schlatter disease .

It can also occur on the back of the calcaneus of the foot. This disease is called calcaneal apophsitis or Sever’s disease. Osteoarthritis may also develop in the metatarsal bones.

Osteoarthritis at the base of the 5th metatarsal is called Iselin’s disease.

Osteoarthritis may also develop in the iliac crest (hip bone ridge).

Iliac crest apophsitis can cause tenderness and pain in the quadriceps or hip muscles. Osteoarthritis of the iliac crest can be treated with rests, ice packs, kidney exercises, physical therapy, and exercise therapy, depending on the site of the osteochondrosis. Sometimes it is treated with a strap or the like.
