골육종 Osteosarcoma – 제1권. 소아청소년 응급의료

골육종 Osteosarcoma

골육종은 뼈에 나는 악성종양이다.
비교적 드문 악성 종양이다.
사춘기 이전이나 사춘기 아이들에게 생길 수 있으나 10~20세 사춘기 아이들과 젊은 성년들에게 더 잘 생긴다.
소아 골격 악성종양 중 가장 흔한 골격 악성 종양이다.
장골 골단에 더 잘 생기고 특히 슬 관절(무릎관절) 부위에 있는 대퇴골 골단에 더 잘 생긴다.
이 종양의 21%에서 야간에 뼈에 통증이 생긴다.
47%에서 사소한 외상으로 골육종이 난 부위에 통증이 생기고 그 부위에서 응어리가 만져진다. 처음 진단 받을 때 39%에서 응어리가 만져진다.
이 병의 주 증상은 골육종이 난 부위에 통증과 부종이다.

골육종 진단

종양이 생긴 부위의 골 부분에서 골격 통증, 근육통, 관절통이 지속적으로 있고
국소적으로 붓고 응어리가 만져지고 운동 제한이 생기면 이런 병을 의심하고 그 부위 골격 X선 사진 검사, 골격 CT 스캔 검사, MRI 검사로 진단한다.
CBC 피 검사, 소변검사, 적혈구 침강속도(ESR), 알칼린인산효소(알카린포스파타제) 혈중 농도나 유산 탈수소효소 등 검사로 진단한다.

골육종 치료

조기에 정형외과 전문의, 악성 종양 전문의 치료를 받아야 한다.
전신의 뼈들과 폐 등에 전이가 되었는지 전신 악성 종양 전의 검사를 한다.
골수에 전이되어 있나 알아보기 위해 골수검사를 한다.

Osteosarcoma 골육종

• Osteosarcoma is a malignant tumor of the bone.

• It is a relatively rare malignancy.

• It can occur before or during puberty, but is more common in adolescents and young adults aged 10 to 20 years. • It is the most common skeletal malignancy among children’s skeletal malignancies.

• It is more common at the epiphysis of the long bones, especially at the epiphysis of the femur in the knee joint (knee joint). • Bone pain occurs at night in 21% of these tumors.

• In 47% of cases, minor trauma causes pain at the site of osteosarcoma and a palpable lump at the site. At first diagnosis, a lump is palpable in 39% of cases.

• The main symptoms of this disease are pain and swelling at the site of osteosarcoma.

Osteosarcoma Diagnosis

• There is persistent skeletal pain, muscle pain, and joint pain in the bony part of the tumor site.

• If there is local swelling, palpable mass, and movement limitation, such disease is suspected and diagnosed with a skeletal X-ray examination, a skeletal CT scan, and an MRI examination.

• Diagnosis is made by tests such as CBC blood test, urinalysis, erythrocyte sedimentation rate (ESR), alkaline phosphatase (alkaline phosphatase) blood concentration and lactate dehydrogenase.

Osteosarcoma treatment

• Early treatment with an orthopedic surgeon or malignant oncologist is essential.

• Perform an examination before systemic malignancy for metastasis to the bones and lungs of the body.

• A bone marrow examination is done to see if it has spread to the bone marrow.
