  • 정삭 축전 회전으로 고환, 부고환과 그 외 다른 조직에 정상적으로 피 순환이 되지 않고, 그 때문에 고환이 손상되고 고환 기능이 상실되는 병을 고환 염전이라고 한다.
  • 음낭 속에 있는 고환은 고환 초막(Tunica vaginalis)으로 고정되어 있는 것이 정상이다.(그림 23참조) 그런데 고환 초막의 길이가 정상 이상으로 길면, 음낭 속에 있는 고환이 음낭 속에서 보다 더 자유자재로 움직일 수 있게 된다.
  • 그래서 고환 초막과 고환에 염전이 생길 수 있다.
  • 정류 고환(Undescended testis)이나 복강 내 잠재 고환, 서혜관(Inguinal canal) 속에 있는 잠재 고환에 염전이 더 잘 생긴다.
  • 이러한 원래 질환이 없어도 고환 염전이 생길 수 있고, 외상 입은 고환에 고환 염전이 더 잘 생길 수 있다.

고환의 염전(고환 염전)의 증상 징후

  • 하복부 급성 복통, 음낭 통증, 구기, 구토, 음낭 발적, 압통 등의 증상 징후가 생길 수 있고, 염전되지 않은 쪽의 고환에 비해 더 크고, 고환 거근 반사(Crematic reflex)가 없어진다.

고환의 염전(고환 염전)의 진단

  • 병력·증상 징후·진찰 소견 등을 종합하여 이 병이 의심되면, 고환 스캔 검사 또는 고환 초음파 검사 등으로 진단한다.
  • 부고환 염전, 서혜부 탈장 감돈, 부고환염, 고환염 등과 감별 진단한다.

고환의 염전(고환 염전)의 치료

  • 응급 수술로 염전된 고환을 풀어 주는 치료를 한다.

• Testicular torsion is a disease in which blood circulation to the testis, epididymis, and other tissues is not normal due to fine shaft rotation, and the testicle is damaged and testicular function is lost.

• It is normal for the testicles in the scrotum to be fixed with the testis vaginalis (refer to Fig. 23). However, if the length of the testes in the scrotum is longer than normal, the testicles in the scrotum move more freely than in the scrotum. You will be able to.

• This can lead to torsion in the testis and in the testis.

• Torsion is more common in the undescended testis, the latent testes in the abdominal cavity, and the latent testes in the inguinal canal.

• Testicular torsion can develop even without these original disorders, and testicular torsion can develop more easily in the injured testicle.

Symptoms signs of testicular torsion (testicular torsion)

• Symptoms such as acute lower abdominal pain, scrotal pain, nausea, vomiting, scrotum redness, and tenderness may occur. They are larger than the testicles on the untorted side, and the testicular levator reflex (Crematic reflex) disappears.

Diagnosis of testicular torsion (testicular torsion)

• If this disease is suspected by taking the medical history, symptoms, and examination findings together, it is diagnosed with a testicular scan test or an ultrasound test of the testicle.

• Differentially diagnose epididymis torsion, groin hernia, epididymitis, orchitis.

Treatment of testicular torsion (testicular torsion)

• Emergency surgery to relieve torsion testicles.
