고부 백선(완 백선) Tinea cruris – 제1권. 소아청소년 응급의료

고부 백선은 트리코피톤 속에 속하는 Trichophyton rubrum이나 Trichophyton mentagrophytes 등 진균이 고부에 감염되어 생기는 피부 백선의 일종이다. 고부 백선을 완 백선이라고도 한다.
완 백선(고부 백선)은 두부 백선, 체부 백선, 족부 백선, 손톱 백선, 결절 백선, 흑 백선, 수 백선, 안면 백선 등 여러 종류의 진균성 피부 감염병 중 하나이다.
완 백선의 가장자리는 비교적 명확하고 모양은 반달형이나 타원형 등이고 외부생식기에 가까이 있는 사타구니 부위, 회음부, 엉덩이, 하복부, 또는 항문주위 등에 주로 날 수 있다.
양쪽 사타구니에 동시 생기는 것이 보통이다.
환부의 색은 붉고 피부 비늘이 붙어있는 경우가 많다.
이병은 주로 사춘기 남아들과 20~30대 젊은 남성들에게 더 잘 생길 수 있으나
어느 연령층 여성들에게도 나타날 수 있다.
육체적 운동을 많이 하는 젊은 남성들, 너무 꼭 끼는 바지를 입는 젊은이들에게도 더 잘 생기고
특히 더운 계절에 더 잘 생길 수 있다.

고부 백선(완 백선)의 진단

병력, 증상 징후, 진찰 소견 등을 종합해서 진단할 수 있다.
그 환부에서 인설을 채취해 진균 세균검사를 해서 진단하든지 우드 등 빛 검사로 진단하든지 진균 배양검사로 확진할 수 있다.
진균 염증성 피부질환이 나은 후 생기는 색소 침착 감소증, 백색 비강진, 백반증, 지루성 피부염, 2차 매독, 장미색 비강진 (Pityriasis Rosea), 건선 등의 피부병과 감별 진단해야 한다.

고부 백선(완 백선)의 치료

ZeaSorb medicated powder 등으로 대증 치료를 하면서 Clotrimazole, Haloprogin, Miconazole, Tolnaftate 또는 Oxiconazole 등의 항진균제 연고나 로션 중 하나를 선택해 치료하거나
국소 항진균 외용약으로 치료가 잘 되지 않으면 Griseofulvin 경구 투여로 치료하면 잘 치료된다(P.000 참조 표1~4 참조).
