가성 안와종양 Orbital Pseudotumor

가성 안와종양 Orbital Pseudotumor

  • 안와 가성종양을 특발성 안와염증 (Idiopathic orbital inflammation)이라고도 한다. 
  • 안와 가성종양은 비특이성 비종양성 안와염증성 질환이다. 
  • 주로 성인들에게 생기나 아이들에게도 가끔 생길 수 있다. 

가성 안와종양의 원인

  • 확실히 모른다.
  • 눈의 근육, 누선, 시신경 등의 염증으로 생긴다.

가성 안와종양의 증상 징후

  • 안구 돌출, 안검 부종, 눈에 통증, 안구운동 제한, 결막수종 등의 증상이 1~4주간 생길 수 있다.
  • 보통 양 눈에 생긴다.
  • 모양체염(섬모체염)도 생길 수 있다.
  • 식욕부진, 복통, 체중 감소, 노근한 증상을 동반할 수 있다.

가성 안와종양의 진단

  • 병력, 증상 징후와 진찰소견 등을 종합해서 진단할 수 있다.
  • 눈 CT 검사, MRI 검사 등으로 감별 진단한다.
  • 박테리아 감염이나 진균 감염, 전이성 종양이나 원시성 종양, 림프종양, 혈관염, 갑상선 안구근육 질환 등과 감별 진단 한다.
  • CBC 피 검사에서 백혈구 증가가 보이고 적혈구 침강속도 검사가 증가되어 있다.
  • 호산구 증가가 생길 수 있다.

가성 안와종양의 치료

  • 전신 코티코스테로이드제로 치료 하고 방사선으로도 치료 한다.
  • 예후는 비교적 좋다.

Orbital Pseudotumor 가성 안와종양

• Orbital pseudotumor is also called idiopathic orbital inflammation.

• Orbital pseudotumor is a non-specific non-neoplastic orbital inflammatory disease.

• It occurs mainly in adults, but can occasionally occur in children.

Causes of pseudo-orbital tumors

• Not sure.

• It is caused by inflammation of the eye muscles, lacrimal gland, and optic nerve.

Symptoms, Signs of Pseudo-orbital Tumor

• Symptoms such as eyeball protrusion, eyelid edema, eye pain, eye movement restriction, and conjunctival hydrocephalus may occur for 1 to 4 weeks.

• It usually occurs in both eyes.

• Ciliitis (ciliosis) may also develop.

• It may be accompanied by anorexia, abdominal pain, weight loss, and senility.

Diagnosis of Pseudo-orbital Tumor

• Diagnosis can be made by combining medical history, symptoms, signs, and examination findings.

• Differential diagnosis is made by eye CT scan and MRI scan.

• Differential diagnosis of bacterial or fungal infection, metastatic or primitive tumor, lymphoma, vasculitis, and thyroid eye muscle disease.

• CBC blood test shows an increase in leukocytes and erythrocyte sedimentation rate test is increased.

• An increase in eosinophils may occur.

Treatment of pseudo-orbital tumors

• Treatment with systemic corticosteroids and radiation.

• The prognosis is relatively good.
